Thank you for agreeing to take part in a study of shopping being conducted by the Social Issues Research Centre (SIRC). We are interested here in shopping for food and household essentials – ‘provisioning’ – rather than for clothes or more expensive household items.
Each fully completed questionnaire qualifies you for entry into a prize draw for one of three Flip Video Camcorders (total prize value £480). The number of people completing the questionnaire will be limited to 100. Terms and conditions can be found at http://www.sirc.org/terms_and_conditions.html.
We estimate that the survey will take around 15-20 minutes to complete. All individual answers will be treated as strictly confidential and will be non-attributable. In order to be eligible for the draw, you must fill out your full name, email address and telephone number on the opening page of the survey so that we may contact you. We will not use your personal information for any other purposes.
In this questionnaire we ask you to indicate your feelings about various supermarkets and shops using scales such as the one below:

If feel that a shop or store is, say, fairly Distant and/or inconvenient to get to, you might indicate this by ticking a box towards the right-hand side, like this:

If, on the other hand, you feel that the shop or store is neither near nor distant; neither convenient nor inconvenient, you can indicate this by ticking the middle box.

If you are not familiar with a particular store or shop, complete the section on the basis of what you think it would be like or, perhaps, what you have heard or read about it.
